Access structured housing placements designed to meet the transitional needs of young adults exiting foster care. Each placement includes oversight, check-ins, and practical guidance to promote a reliable environment. Participants are matched with available housing based on eligibility and availability criteria. Case coordinators support housing services to ensure participants uphold their responsibilities and participate in wraparound services. Referrals and transitions are coordinated with social workers and partner agencies.

- Transitional Housing Placement Program for Non-Minor Dependents (THPP-NMD)
The THP-NMD provides structured housing and support services for eligible youth ages 18 to 21 who are participating in Extended Foster Care. This program offers supervised settings and case-managed support to help participants build daily routines, pursue education or employment, and meet program expectations.
